The short answer: Yes, and less than the hype suggests. Some of the questions that used to begin a search now end inside an answer from ChatGPT, Google's AI Overviews, Gemini, or Perplexity, and a practice the paragraph skips is out of that decision before anybody scrolls. It bites harder in dentistry because the two things people lead with, whether you take their plan and whether they can be seen today, are exactly what an answer engine will settle on its own. It belongs inside ordinary search work rather than a separate program, and nobody can promise you a place in the answer.
Every local business has a version of this problem. Dentistry has features that make it sharper.
None of this makes AI search the main event. Plenty of patients still open the map and call. But health questions come back hedged, and a hedged answer reaches for whatever looks accountable, which favors a practice that has written its facts down. The shift is not that people ask different questions. It is that they stop typing three words, describe the whole situation, and ask who to call. What comes back is a paragraph, and the practices named are the ones that put the answer in writing first.

This part moves fast, so plainly: Google now sells ad placements in and around AI Overviews and AI Mode, and other engines are testing the same. Anyone telling you AI answers are an ad-free zone is reading from last year's briefing.
What has held still is narrower and more useful: no ad budget decides which practices the organic answer itself names and recommends. You can buy a placement in and around the answer. You cannot buy the sentence naming which two dentists in town see anxious adults. That sentence gets assembled from what these systems can read about you, which is why the recordkeeping above is the job.
Paid search is close to the opposite, buying visibility the same afternoon. For most practices both belong in the plan, and Google Ads for dentists covers that side.
None of this needs its own program. The order matters more than the effort.

Probably not much this month, and a great deal in three years. A full schedule usually means recall is doing its job, which is a different engine than new patient acquisition and good at hiding a slow decline in it.
Sometimes, and often not directly. Somebody reads an answer naming your practice, then searches your name, so the visit lands in analytics as branded or direct traffic. Anyone quoting you a confident number for this is guessing. The front desk question, how did you hear about us, is still the better instrument.
It helps, if you are specific. Out of network is where a vague site loses people, because somebody else answers the coverage question for you. Say what out of network means here, how treatment gets quoted, and what payment options exist, and an answer engine has something accurate to repeat.
