Google Ads for chiropractors, managed week by week

In most towns a handful of practices bid on the same short list of searches, and the account somebody watches beats the account somebody set up once. Ads can have your practice above the organic results within days, and they can just as easily spend a month's budget on people looking for a massage, a job, or a degree program. Ours is the watched kind: built around what a new patient is actually worth to your practice, with a flat monthly fee quoted after a free consult.

Why chiropractic ad accounts quietly overspend

These accounts rarely fail loudly. They spend steadily on the wrong clicks while the schedule stays light, and the monthly total looks the same either way.

What hands-on management covers in a chiropractic account

The work is not one setting. It is a set of small decisions, revisited every month.

The monthly report says what it cost, what came in, which searches did the work, and what we changed. Nothing that requires a decoder.

What a new patient is worth, and why that sets the budget

Before anyone picks a budget, one number has to come from you: what a new patient is worth to your practice. Not a figure from an industry webinar. Yours.

The parts of it are the share of new patient exams that go on to a plan of care, the average value of that plan, and your mix of cash and insurance. A cash package and an insurance visit sit at opposite ends of the same account and support very different click prices. Reimbursement timing counts too, since money that arrives in ninety days funds advertising differently than money collected at the desk.

Once that number exists, the rest is arithmetic: clicks per call, calls per booked exam, exams that continue as care. Narrower searches, a service plus your town or a specific insurance question, usually cost less and come from someone closer to booking, though there are fewer of them. Broad near-me searches cost more and convert unevenly. Most accounts want both, and your numbers decide the split.

Your calendar belongs in the plan as well. If your practice sees a January stretch when deductibles reset, a fall bump when school sports start, or a seasonal swing in your area's population, the budget should lean into those weeks instead of sitting flat all year. You know that calendar better than any planning tool does.

Compliance is part of the account, not an afterthought

Healthcare advertising carries more rules than most local services. Worth knowing where they come from before anything goes live.

That discipline runs through the rest of our work too. We built a scripted chat concierge for a Marco Island boat-tour company, answering guest questions across roughly 500 pages with owner-approved answers only, because an invented answer is worse than no answer. The rule applies harder in a practice.

Paid search next to SEO, told straight

Ads rent attention for as long as the card is charged. Rankings and a strong profile keep working after the spending stops. Neither replaces the other, and running ads does not lift your organic position or your placement in the map pack. Treat any pitch that claims otherwise as a warning sign.

What paid search does contribute, beyond visibility this month, is the search terms report: a plain record of how patients in your area actually phrase what they need, which is the best list there is of pages worth writing.

More people now read answers without clicking anything, which is what our AI search work covers. The whole picture for practices sits on the chiropractor overview.

Why this is not a template account

Plenty of agencies run one campaign template across every client in a vertical, bill a percentage of the budget, and send a dashboard nobody reads. That model scales for the agency. It is also why so many owners describe Google Ads as something they tried once.

Starting an account, and what the fee looks like

Four steps, whether the account is new or inherited from whoever had it last.

Agencies price this two ways. One is a percentage of ad spend, which quietly rewards them for talking you into a bigger budget. The other is a flat monthly management fee that does not move with the budget. We use the flat one and quote it after the free consult, once we know what the account will genuinely take. We publish no number for it, because a single-doctor office and a three-location group are not the same job.

Ad spend is separate and stays on your own card, never marked up. If the practice needs a landing page or a full rebuild, a custom-coded build runs $3,500 to $12,000 or more, one time, and we will show you a free mockup before you commit. Ongoing organic work runs $1,500 to $3,500 a month for most practices, $3,500 to $7,500 in competitive metros or across multiple locations, explained in how much SEO costs.

Common questions

Can you guarantee a number of new patients?

No. Google Ads is a live auction, and demand, competitor bidding, and your own intake all move the result. We will not put a patient count or a cost per new patient in writing, and we would be wary of anyone who does. What we commit to is a carefully built account, waste cut every month, ad copy you approved, and honest numbers in the report.

What do you charge to manage a chiropractic Ads account?

We quote a flat monthly fee once the free consult shows what the account needs. The other common model in this industry, a percentage of ad spend, pays the agency more whenever you spend more, which is exactly the wrong incentive. Scope sets our number: one office with two services is a different job than three locations. Either way it stays month to month, with no long-term contract.

Are there rules about what a chiropractic ad can say?

Yes, from two directions. Google's healthcare policies limit certain claims and restrict targeting built around health conditions, and some healthcare categories require certification before they can advertise at all. Separately, state boards regulate professional advertising, including testimonials and free exam offers. We are not your compliance or legal advisor. You and your counsel set what is allowed, and you approve every line before it runs.

Should we bid on auto accident or personal injury searches?

Only with your eyes open. Those searches attract law firms and larger clinics, so clicks are among the priciest in the category, and whether they suit you depends on your case mix, your intake process, and your state's rules on soliciting accident patients. If you want to test them, we start small, watch the search terms closely, and stop if the arithmetic does not hold.

Will running ads improve our map pack ranking?

No. Paid placements and local map rankings are separate systems, and buying clicks does not move your Google Business Profile position. Ads sit above the map for some searches, which can help visibility while the local work builds, but the ranking itself comes from the profile, your site, reviews, and consistent listing data. We treat them as two projects that support each other.

Do we own the account and the data?

Yes, all of it. The Ads account lives in your practice's name from day one, or moves there if we set it up. Campaign history, conversion data, the website, the content, the tracking: all yours. Nothing is held hostage if we part ways, and whoever manages the account next inherits it clean and documented.
